Required Information for Initial and Modification Filings
Dissolution of Marriage Petitions. Oregon law requires that certain information be stated in the initial petition for dissolution of marriage. A petitioner also is required to complete the Oregon Vital Statistic form. If the dissolution petition asks for custody or parenting time of a minor child, additional information required by the UCCJEA also is required.
Modification Filings. If the modification involves the custody or parenting time of a minor child, information required by the UCCJEA is required. A Vital Statistics form is not required. If the modification asks to change spousal or child support, then a Uniform Support Affidavit is required. This form is set out in a separate page of this web site.
Other Filings. Any other filing that involves the custody or parenting time of a minor child is required to state the information required by the UCCJEA. This includes a petition for custody and parenting time of unmarried parents, a petition by a psychological parent, and a paternity or filiation petition. A Vital Statistics form is not required
